I also have a Rev C board but can load either the binary or the S19 file with the same results. However since the code has been loaded and is basially running I don't thik that this is a major issue; just strange because I work only with binary files on 12 different FRDM boards and never had a problem.
I note that you posted my code as S19 but it would be more interesting to try to run the code that you built to verify that it operates on my board (the s19 that you attached loads and runs correctly on my board - I get a keyboard, mouse and hard drive operation).
I am trying to interpret whether the USB is operating on your board or not - these are the checks that can be done:
1. Green light blinks at 2.5Hz (I think that this is OK)
2. In the SLCD on the board the ':' sign also blinks at the same speed.
3. When the USB connector is attached to K46USB the device enumerates and "USb" is displayed in the SLCD
4. If you press either SW1 or SW3 the mouse will move left and right
In the case of the harddrive - the PC will normally show a dialog that the disk needs to be formatted (pops up after a few seconds) but it depends on the Windows version and might have to be searched for under "This Computer" or something similar.
The disk should however always be visible in the device manager under hard drive:

Note that also the loader of the FRDM-KL46Z is seen there.
Basically, if you see the "USb" in the SLCD and the mouse operates I would say that USB is OK. If not it means that the USB interface on your board is not operating correctly.
