ADC question: continuous conversion with hardware trigger for k60 ?

I tried to do ADC continuous conversion with hardware triggers, based on AN4590:

Unfortunately, I only observed one conversion with one trigger, even though continuous conversion is enabled. (ADCO = 1)

Can somebody provide sample code for doing ADC continuous conversion with hardware trigger?
