First time programming imx rt1060

I'm programming a custom board with an RT1062. I believe this is a version of the RT1060 that does not include internal flash memory, and uses external flash instead (using NOR SPI Flash specifically).

I'm wondering if there are any pitfalls when programming this processor for the first time. I'm getting the following errors when trying to program the board

Unable to connect wire for probe index 1
Error: Wire Ack Fault - target connected?

02: Failed on connect
Could not connect to core
31: No connection to chip's debug port

Failed on connect: Ee(42). Could not connect to core.

These errors seem consistent with the processor not receiving the correct voltage or unable to communicate over the SWD interface. However, I've verified the voltage coming into the processor at its various Vdd/NVcc inputs is correct using the Hardware Design Guide. 

I've also verified the boot config/boot mode GPIO connections are correct (Internal Boot, Auto-Probe disabled, boot device set to Serial NOR) . I'm scratching my head as to what other causes there could be for this issue.

Yes, the board is a revision to a previous design, so I think the problem is more likely to be a process issue, rather than a design one. I didn't burn any of the fuses, we instead configure the processor using GPIO pins. So far I have verified:

Voltage at the power rails. I used the hardware design guide for reference.

Expected resistors present for boot config/boot mode.

POR input receives high signal from watchdog IC, and reset switch drives signal low when pressed (WDT is active low). So there seems to be no RESET line being held down.

Verified power up sequence requirement... DCDC_PSWITCH is at least 1 ms after DCDC_IN (actual is more like 40 ms). POR_B signal becomes high after PSWITCH does.
