Unfortunately it is hard to say what could be reason for that behavior.
I would like to recommend double check connection on both sides of BDM ribbon cable.
Pin1=BKGD, pin2=GND, pin4=RESET, pin6=VCC.
The same has to be checked also on your board schematic/layout. Please be careful with top and bottom connector orientation…
I would like to recommend also try different power supply adapter with higher current capability.
BDM connection to MCU typically means increasing of power supply consumption (in mA range).
Is it possible measure board power supply current with and without BDM connected?
If you use laboratory power supply, are you sure that current limiter do not influence result voltage?
You could try also different AC plugs. I already meet with situation where ground currents between two power sources cause problems during programming…
You could try different PC or BDM Multilink for at least localize source of that issue.
