NFC Tag Writer App

I'm using the NFC Tag Writer to write ID wristband tags with unique idenitfiers (15-digit text field).  It works fine manually, but I'm having trouble uploading a CSV file so I can write 1000+ tags easily.  Does someone have a sample CSV file that works?  I only need 1 field per line.  This is critical for an event we are having next week. 

Message: "No NDEF records found."
