What does "02: Failed on Connect Em(04). Cannot provide power to DAP bus." mean?

I have a new layout of my board and I get this message when I hit "debug".

We started having problems with 4 LPCXpresso boards along with our two target systems.  We found the solution at:

In real terms, you have a Debug Port (DP) related connection or power problem.

This error means a read of the DP CTRL/STATUS register has determined there is a problem with the DP clock or power domain(s). This is one (or more) of CDBGPWRUPREQ, CDBGPWRUPACK, CSYSPWRUPREQ, CSYSPWRUPACK, or ORUNDETECT.

You can refer to the ARM document "ARM® Debug Interface v5.1" (DSA09-PRDC-008772) for more information.
