in CW debugger, it sometime shows "Trigger A & B occurred"
what does it mean??

I have seen this message when the power on the target is interrupted, or the bdm connection is intermittent.

That message is generated when you have enabled some triggers on the DBG module.
In the connection menu (MultilinkCyclonePro, GDI, .....) select "Trigger Module Settings".
 
There are probably some triggers defined in the Trigger settings tab.
Delete the triggers if you do not want to see the message again.

Hi
or you have triggered an interrupt but the vector setting is not correct! I.E. there was an TIM OC interrupt but you have forgoten to set the VECTOR x (in the prm file) to your interrupt routine.
