Question about processing when not using USB with LS1012A

We made custom board using LS1012A.
We don't need the USB interface, so we conform to the following description in AN5192 (Rev.1).
    Table 3. Power and ground pin termination checklist
    Table 31. USB1 pin termination checklist

Now, our board does not bootup.
We face a similar phenomenon when without using USB with LS1012ARDB.

In addition to the hardware design of AN5192, could you advise us the points we should take attention to?
