3 SEAT LICENSE

We purchased a 3 seat license, and we're having trouble getting the 3rd seat to work.
It says we've exceeded number of licenses. 
Can 1 computer have used 2 seats by upgrading from LPCXpresso_6.1.0_164 to LPCXpresso_6.1.4_194?
If not, then how to we register a 3rd seat when it says we've used all 3 seats, but we haven't?

Can you remove TK42? It was on the same computer as KMATTHEWSASUS, but the computer was renamed and he added the license key again by accident.

Because you HAVE activated 3 times, on 3 computers. The computer names are
KMATTHEWSASUS
