A probability of USB hub failure when a 3.0 udisk was inserted on custom board

When the system is booted with a 3.0 udisk was inserted on my board, there will be a probability of USB hub failure. When this happens, all USB interfaces from HUB can't be used.

Normal：

# lsusb

Bus 001 Device 005: ID 0951:1666

Bus 001 Device 002: ID 0424:2517

Bus 001 Device 001: ID 1d6b:0002

Abnormal：

# lsusb

Bus 001 Device 002: ID 0424:2517

Bus 001 Device 001: ID 1d6b:0002

SW version as follows：

BSP version :Yocto 2.1 

Kernel version: 4.1.15

But there is no such phenomenon on Yocto 1.8(Kernel version:3.14.52).
