Hi,
Transistor wants to use DC-DC converter to run a fever.
May it supply 1.2V to VDD directly using DC-DC converter without using external NPN ballast transistor(BCTRL)?
Please give me ISSUE of that case.

That's a nice app note. I'm surprised that it doesn't show the A5 at its full capabilitiy of 500 MHz as opposed to the listed 399 MHz.
Are there any issues/concerns with running the A5 at 500 MHz?

No, there are not as long as it is mentioned in the Datasheet.
