Power and IO shorts spontaneously developing on P1021

We have seen several field failures of a product containing the P1021 processor.  Apparently spontaneously, while the system is up and running, the system shuts down.  Board level analysis shows dead shorts to ground of the 1.0V and 3.3V rails.  Upon removal of the processor, dead shorts are found on the Vddc (Core 0 and platform supply), the OVdd (IO subset supply), and on 8 or so IO pins in a tight cluster in the vicinity of the OVdd pins.

This would appear to be the result of some kind of over-voltage stress, but we can not find an obvious cause.  Does this seem more likely from an event on the OVdd line or on one of the failing IOs?  Note OVdd, BVdd, and CVdd are all tied to the 3.3V rail, and only IO associated with OVdd are found with shorts.  No other parts on the board are found to be damaged - many others share the 3.3V rail.  No other devices share the 1.0V rail.

Any thoughts on possible causation given these symptoms?
