I have done a test to connect the LPC55S69-EVK board with the MCU-Link Debug Probe board, they work fine based on the MCUXpresso tools and SDK LED example.
Pls Build and Debug as the following screenshot from tools perspective.

If you still have issue, can you tell us if this is the first time you try to communicate with customized board? If it is, pls post the schematics so that we can have a review if it is convenient, because I suspect that your board has issue for example SWD pin assignment, power supply part...

Answering the Questions u have asked
1)I have been using this board for quite a long time, more than a year now
2)I don't suspect any issue with the LPC55S16JBD100 as the board is still having the power confirmed with the LED and also the logs are been seen from the previous built, we had flashed previously
3)As u mentioned there can be power issue, but according to my understanding power issue is been is there on the debugger as there is no power led on, if possible can u share me the pin out for the MCU-Link debug probe to provide external power source and check with it
