Bug: KDS fails to start debug with "Error in final launch sequence"

K64F Freedom board, latest KDS (all updates as of last week).
Debug was working OK and suddenly I get the following error when trying to launch debugger:

If I power-cycle the board I can drag-and-drop a "hello world blinky light" application into the mbed drive and it works, so the board seems to be OK. There are no zombie debug processes (the problem happens every time on clean start of KDS and board).

Anybody have any idea what's going on?
