I believe that as same as customer, you overlooked the boot_cfg switches. As the image attached on the post. SW2 wich is Boot_cfg needs to be b0100....
This is first done by hardware before burning any fuses to check that the boot configuration is correct.

I am working on a RT1165 custom board and attempting to enable authenticated OTFAD using the Secure Provisioning Tool v25.09. Flashing plain images and dual boot work as expected but with OFTAD enabled the device boots straight to ISP mode.

It seems to be similar with this issue: Solved: RT1170 OTFAD Troubleshooting - NXP Community

But I do not see "FlexSPI NOR + Encrypted" options in the user manual (IMXRT1160RM.pdf starting pg 263). And ENCRYPT_XIP_ENGINE is managed by the tool.

Is there another register/fuse/GPIO that needs to be set from the secure provisioning tool for the RT1165 to boot OTFAD encryption?

OTP

settings

Thank you for the reply! Is it possible to force the OTFAD boot with fuses regardless of the hardware switch settings?

I will answer my last question. Yes, you can set the fuse manually regardless of the switch configuration.

Just note that the Secure Provisioning Tool does not have a label for that fuse.

Set as above and the board will now boot with OTFAD encryption.
